When a nonconformity occurs it's critical to take action to control or correct it in a timely manner. Your business must evaluate the need for corrective actions to identify and eliminate the root cause(s) to avoid reoccurrence.
This 5 page 5 WHY Analysis Form has been developed to assist with identifying the root cause, or causes, following a nonconformity or incident. A series of tables ask you "WHY" until a root cause is established, and a helpful example scenario is provided to show how easy it can be. Management of Actions can also be recorded including corrective actions and preventative measures .
